Start at the bottom (2.5 mg) and titrate up. Do not try to convert your semaglutide dose to an "equivalent" tirzepatide dose—they are different drugs with different mechanisms. Some members who aren't prone to side effects start at 3.5 mg after being on max-dose sema, but this is the exception.
